The Oklahoma State Department of health requires the forms below to be filled. Including Temporary License fee $50 (All links listed below) Affidavit of lawful presence. Temporary tattoo license, CPR certification. CHSLicensing@health.ok.gov
10x10 booth: $700 single booth $1200 for double booths. 2 artists per booth. Add all attending artist names below if sharing. Payment link will be sent separately after approval. Booth will include complimentary paper towel roll, distilled water, 6x6 table per booth, 2 standard chairs, souvenir Artist lanyard, 1 complimentary weekend wristband for booth help, signage hooks, sharps container, trash receptacle.
