The Oklahoma State Department of health requires the forms below to be filled. Including Temporary License fee $50 (All links listed below) Affidavit of lawful presence, Temporary tattoo license, CPR certification.
Fill out as much as you are capable of. The Health Dept is willing to work with you with any missing paperwork or with any questions you may have. Email paperwork at CHSLicensing@health.ok.gov or use the portal below. “Oklahoma State Dept of Health Link”

* We are more than happy to assist Out of state/Country artists. We can deduct your course and Temp License fee if you send us a receipt of purchase.
*We were notified by the OK State Health Dept the paperwork does NOT need to be notarize.
10x10 booth: $700 single booth $1200 for double booths. 2 artists per booth. Add all attending artist names below if sharing. Payment link will be sent separately after approval. Booth will include complimentary paper towel roll, distilled water, 6x6 table per booth, 2 standard chairs, souvenir Artist lanyard, 1 complimentary weekend wristband for booth help, signage hooks, sharps container, trash receptacle.
